The Science Of Pleasure: Part III- The Neurological Orgasm

When we last spoke about sex, we discussed the neurotransmitters involved in pleasure and attraction, namely dopamine and oxytocin. Now let's look a little deeper into the action of those neurotransmitters and how we can manipulate their action- to extend the neurological orgasm for as long as possible.

The dopamine-oxytocin response does not follow an "all or nothing" rule. Some stimuli will invoke a huge, quick response, and others a smaller, yet longer, extended response. There is a varying degree of neurotransmitter release depending on the strength of the stimulus, as well as how long we are in the presence of it. Remember last time- I told you that the longer you interface with a stimulus, or the more time you spend interacting with it, the greater attachment you will form to it.

Some studies have looked at the different types of sex and the variable neurochemical responses based on the type of sex the person is engaging in. However, it is probably not the case that the penis causes more dopamine to be released than a vibrator, for example. More likely, it is the state of mind the person is in that really has an effect on the amount of pleasure he or she is experiencing.

The cognitive state of the brain at any given moment in time is what causes the release of dopamine, whether it is prompted by a physical or a mental stimulus. Can the brain be stimulated via tactile nerve pathways to release large amounts of dopamine? Sure. However, consider this: You can give one person two separate, yet identical patterns of physiological stimulation, and end up getting radically different subjective experiences as a result. (1) This implies that it is not just the physical act of sex that gives us so much pleasure, but how we represent that act in our mind that really has impact.

Not only can we get just as much of a neurotransmitter response from visual input or representational input (fantasies) as we can from physical input, but sometimes it can be even better than the real thing. The key to inducing that type of neurotransmitter response is the strength of the stimulus. When I say "strength" I don't necessarily mean a strong, direct stimulus, like a nude photo, for example. Yes, a nude photo will probably get you a huge dopamine spike. But it is relatively short-lived, like a quick orgasm. We can do better.

There are ways to trigger a dopamine response that may not be as high initially, but it lasts longer, thus giving you more time to interface with the stimulus, feeling that release of oxytocin as well- and making it extend for as long as possible. Why would we want to do this? To get hooked on a feeling, of course! *cue Blue Swede* Plus... we just enjoy it longer. More time spent happy = good.

So if you want to have the most prolonged neurological orgasm possible, how could you achieve that?

Let's look at some examples from photography used in advertising that are designed to do exactly that- titillate and engage the viewer for as long as possible. They employ certain techniques that aid to this goal: suggestion, ambiguity, anticipation, and telling of a story.

Here we have some visuals from Dolce&Gabbana, who are notorious for their controversial and sexually charged ads.

So here we have Scarlett Johansson, posing for Dolce&Gabbana, looking ravishing as ever. What is it that makes this ad so sexy and dopamine-triggering? Well, several things. First of all, she is positioned in a bed, lying down, which makes us think of sex (through learned associations). Easy enough. She also has tousled hair and appears that she has just engaged in sexual activity of some kind- brilliant.

What they did was prompt the viewer to engage in a fantasy involving Scarlett- they put all the right elements there to start the story- in a bed, wearing lingerie, pre or post copulation, and we fill in the rest. We are brought into the fantasy state, imagining the sexual scenario, and we feel the rush of dopamine and oxytocin. Now we are spending more time thinking about what she was doing, who it was with, possibly inserting ourselves in the fantasy.

Here is another great ad that has oh, so many genius design and psychological elements going on. The couple seen here is in a romantic embrace, just about to kiss. The fact that they are about to kiss (and haven't yet) is pretty important. We are completing that sexual scenario in our mind, filling in the blanks, lingering on the subject matter. We are spending more time thinking about the subjects, feeling what they are feeling.

The longer foreplay of images prolongs the release of dopamine and oxytocin, and basically creates an extended orgasm in your brain. A blatant sexual image induces a large, quick shot of dopamine, but anticipation and suggestion of sensuality produces a much longer pleasure response. We don't want the image to do all the work for us; we want to travel down that road in our mind ourselves so we can enjoy the ride.

Why is ambiguity so arousing?

Mystery triggers dopamine. When it comes to sexual innuendo, people want to be brought up to the line as close as possible, without crossing the line. The "line" is actually the point at which it is obvious to the viewer where you are leading them (to imagine sexual activity), but you don't bring them too far, so that they miss out on the pleasure of the experience.

People also generally like feeling in control. If you give enough suggestion- without going too far- which can be a turn off, you allow them to have the illusion that they are choosing their mental path, when in reality, you are leading them there on a leash. We physiologically and mentally like the path of sexual innuendo, we want to be on that path. Yet we want to feel like we are there of our own volition.

Our brain will see ambiguous images, for example, in the D&G ads- partially clothed woman, with messy hair and parted lips. These are all images that cue sex in our brain, because we know that those elements are logical precursors to sex. When our brain picks up these cues and recognizes the "sex schema", it fills in the gaps and continues to play out that schema in our mind, living that moment chemically as well. We don't have to be living the moment physically to experience these chemical benefits; visualization- if intense enough- can do this, which is why fantasies are so sexually powerful.

Beginnings and Endings- Telling the Story

When we see an image in a sequence, we are automatically inclined to finish the sequence. Just like the nature of forward momentum is to complete a step that we begin taking, so are we mentally driven to complete a story that has been started. When a sexual scene is pictured at a mid-point of activity, we are compelled to finish out the play, to take the elements to their logical conclusion.

The same phenomenon occurs when we are shown a series of images with gaps in the story. Film makers and video directors are masters at this. For example, in Lady Gaga's video, Alejandro (particularly at ~2:50 and beyond), never at any point are we shown actual sexual penetration of any kind. We are shown flashes of scenes, with just enough suggestion to let us know what is going on, but enough ambiguity to allow for our brain to spend time on it, filling in the blanks. By doing this, we are brought into the story- no longer just bystanders merely watching, but active participants in the sexual scene, filling in the details to our own specification, which makes it all the more powerful.

So what are the take home points? Imagery and suggestion that is blatant and forceful may get a big spike in dopamine, but it will be short-lived. This isn't giving you enough time to feel the attachment effects of oxytocin to the fullest extent possible. The element of mystery is a trigger for dopamine- in figuring out that mystery, we are intrigued, drawn in, motivated to examine it closer. If a stimulus is more abstract and leaves a little to the imagination, you will spend more time on it, thus extending every pleasurable neurochemical process, giving you a prolonged neurological orgasm. Bottom line? A little mystery is a good thing. Trust me- you'll enjoy it more.

Erectile dysfunction affects an estimated18 million men in the United States, with a prevalence of 18.4% in men aged 20 years and older. Prevalence increases with age, ranging from 5% in men aged 20-39 years to 70% in men aged 70 years and older. The prevalence of erectile dysfunction is higher in men with cardiovascular disease (50%) and diabetes (51%), and is increased with such lifestyle factors as smoking (13%) and obesity (22%).

Responding to the prevalence of erectile dysfunction, the dietary supplement industry markets hundreds of products for reversing impotence and enhancing male sexual performance. Legally, dietary supplement labels cannot make medical claims, such as "for treatment of erectile dysfunction"; however, such claims as "to enhance sexual function" are permissible. An Internet search for "male sexual enhancement products" yielded more than 2 million hits, with websites offering products for purchase as well as information and testimonials.

Labeled Ingredients

Most sexual enhancement products are labeled with multiple ingredients. Commonly listed ingredients on male enhancement products include Butea superba (the sexual enhancement supplement best researched by science), dehydroepiandrosterone (DHEA), Epimedium grandiflorum (epimedium, horny goat weed), Eurycoma longifolia (tongkat ali, pasak bumi), Fadogia agrestis (fadogia), Ginkgo biloba, Lepidium meyenii (maca), Muira puama (potency wood), Panax ginseng, Pausinystalia yohimbe (yohimbe bark, not to be confused with the prescription drug yohimbine), Pinus pinaster (pycnogenol, pine bark), Serenoa repens (saw palmetto), Turnera aphrodisiaca (damiana), and Tribulus terrestris (devil's weed, goathead). Vitamins, minerals, and amino acids, such as L-arginine and propionyl L-carnitine, are frequent additions.

Many of these products have been studied only in male rats, but the few studies in men have been small or poorly designed, limiting conclusions about efficacy and safety.

Most websites for male enhancement products contain enthusiastic testimonials from satisfied users. But the question remains of whether these products really work, despite the dearth of clinical evidence supporting the efficacy of the ingredients.

Unlabeled Ingredients

Some products for sexual enhancement augment sexual activity, but the labeled ingredients may not be the source of the effect. Of the 232 drug recalls by the US Food and Drug Administration (FDA) between 2007 and 2012—all for unlabeled drug ingredients—51% were dietary supplements. Of the dietary supplement products recalled, sexual enhancement products were the most commonly recalled (40%), followed by bodybuilding (31%) and weight-loss products (27%).[7] Of the 1560 Health Safety Alerts for dietary supplements issued by the FDA MedWatch and Health Canada between 2005 and 2013, 33% were for sexual enhancement products.

Unlabeled drugs in sexual enhancement products are frequently the prescription-only phosphodiesterase 5 (PDE5) inhibitors, such as sildenafil (Viagra®), tadalafil (Cialis®), vardenafil (Levitra®), and avanafil (Stendra®). With increasing frequency, the unlabeled drugs may be analogues of PDE5 inhibitors that have been modified slightly from the parent structures. These derivatives are not detected by routine laboratory screening, which reduces the risk for both detection by the FDA and lawsuits for patent infringement.

To date, more than 50 unapproved analogues of prescription PDE5 inhibitors have been identified.

Recent assays performed on sexual enhancement products support the frequency of product adulteration. Of 91 products analyzed, 74 (81%) contained PDE5 inhibitors, including tadalafil and/or sildenafil (n = 40) or PDE5-inhibitor analogues (n = 34). Of the products containing prescription ingredients, 18 contained more than 110% of the highest approved drug product strength.

Another study of 150 sexual enhancement products (eg, Evil Root, Herbal Stud, Magic Sex, ULTRASize) found 61% of the products were adulterated with PDE5 inhibitors: 27% with sildenafil, tadalafil, or vardenafil, and 34% with similar structural analogues. Among the adulterated products, 64% contained only one PDE5 inhibitor and 36% contained mixtures of two to four PDE5 drugs or analogues. The amounts of PDE5 inhibitor prescription medicines were higher than the maximum recommended dose in 25% of products.[8] Unlabeled yohimbine, flibanserin (Addyi™, which was recently approved by the FDA for female sexual dysfunction), phentolamine, DHEA, and testosterone also were found in some supplements.

Other researchers have found similarly adulterated products, many containing PDE5 inhibitor doses in excess of labeled amounts.

Safety Concerns

Although dietary supplements are marketed as "all natural" with implied safety, the available research suggests caution.

A recent survey indicates that cardiac symptoms were a frequent cause of emergency department visits among men aged 20-39 years taking sexual enhancement products.[14] The actual prevalence may be higher, because the presence of unlabeled PDE5 inhibitors may easily go unrecognized by clinicians. Common adverse effects of PDE5 inhibitors, such as flushing, lightheadedness, or dyspepsia, may be attributed to niacin and yohimbe, ingredients often found in sexual enhancement products. Profound hypoglycemia after ingestion of sexual enhancement products containing sildenafil and glyburide (Micronase® and others) also has been reported.

The covert addition of analogues of PDE5 inhibitors, which are not readily detectable by chemical screens, is particularly concerning. Although these chemical cousins of PDE5 inhibitors may retain the desired pharmacologic effect, none have been clinically tested for safety and toxicologic effects.

Obtaining dietary supplement products for sexual enhancement products has several perceived advantages. The purchase can be made discreetly, conveniently, and without a visit to a prescriber. Unlike drugs, dietary supplements are not required to be labeled with adverse effect or drug interaction information. Men taking prescription drugs, such as nitrates, may perceive dietary supplements for sexual enhancement as safe alternatives to contraindicated PDE5 inhibitors.

Clinicians should maintain a high degree of awareness for the potential for adverse effects of sexual enhancement products in men with unexplained cardiovascular symptoms. Patients who express interest in sexual enhancement supplements should be referred to their healthcare provider. Explain that even though a PDE5 inhibitor is not on the label, the supplement may have these ingredients added illegally without regard to patient safety. Patients should be warned of possible changes in vision and decreases in blood pressure, and the potentially dangerous combination of PDE5 inhibitors and nitrates that require medical advice.

PDE5 inhibitors are substrates of cytochrome P450 3A4 (CYP3A4). Monitoring is required to avoid an interaction with CYP3A4 inhibitor drugs, such as erythromycin, which may result in high PDE5 levels.

In summary, advise patients that dietary supplements for sexual enhancement fall into one of two categories: those that might be safe but do not work, and those that might work but are not safe.

Pedophile suicide linked to McCann

A PEDOPHILE who was on holiday in the Algarve at the time British toddler Madeleine McCann was taken from her bed has been found dead after abducting a five-year-old girl.

Urs Hans Von Aesch killed himself in a Swiss forest close to the belongings of Ylenia Lenhard, who is still missing a week after she disappeared.

Portuguese detectives have been told of resemblances between the abductions of Madeleine and Ylenia, who look remarkably similar. They have also been informed that Von Aesch, who lived in Spain, was thought to have been close to Praia da Luz when Madeleine disappeared 96 days ago.

The development came as Portuguese police found traces of blood in the bedroom where Madeleine had been sleeping with her two-year-old brother and sister, Sean and Amelie.

Tiny specks of blood were discovered on a wall by a sniffer dog that was taken to Portugal by British police. The blood has been sent for DNA analysis.

Von Aesch, 67, is regarded by Swiss police as the key suspect in the abduction of Ylenia, who disappeared while walking to a swimming pool in the northeast of the country on Tuesday last week.

The girl's clothing, packed in her backpack, was discovered in a forest 32km from where she was abducted. Von Aesch shot and injured a 46-year-old man in the same forest before killing himself on Tuesday last week.

Hans Peter Eugster, a Swiss police spokesman, said: "Because of the similarities in the case and because Von Aesch lived in Spain, we have informed Interpol.

They in turn have passed on details to colleagues in Spain and Portugal and they are looking at reports that Von Aesch was on holiday in the Algarve."

Bruno Fehr, the chief of St Gallen state criminal police, said that the search for Ylenia continued yesterday. "We haven't given up hope. But we have to expect the worst now," he said.

Von Aesch, who had lived with his Spanish wife in the town of Benimantel for 10 years, drove a white van similar to one that was seen outside the McCann's apartment in the resort when Madeleine disappeared.

A Portuguese police team led by two British officers has finished a second search of the home and garden of the only official suspect in the case, Robert Murat. Tuck Price, a spokesman for the Murat family, said that the police had not discovered anything of interest while searching the villa.

Mr Murat, 33, has insisted that he was at home with his mother, Jennifer, on the night that Madeleine went missing. Mrs Murat, 71, said: "He's going to be cleared, he is innocent."

Yesterday police removed several vehicles, including Mr Murat's car, his mother's camper van and a van belonging to the Ocean Club resort for tests by forensic science experts.

Portuguese and British police have also been monitoring a British tourist who was on holiday at the Ocean Club resort when Madeleine disappeared.

The man, who has since returned to Britain, is under suspicion because of contraditions in witness statements he gave to police.

A source close to the Portuguese police investigation said: "Murat was not the only person in the frame. Another man has been under surveillance for a period of time but has no idea that he is being watched."

Until now, the strongest evidence provided to police has come from Jane Tanner, who was on holiday with Madeleine's parents, Kate and Gerry McCann, and her partner, Russell O'Brien.

Ms Tanner, 36, from Exeter, said she saw a man walking away from the McCanns's appartment carrying a girl.

Swarez, of Crosby in Merseyside, was jailed for 17 years in 2012 after Liverpool Crown Court heard how he was the ‘managing director’ of a drugs gang responsible for flooding the UK with heroin and cocaine.

Swarez pocketed £1.5m from his part in the gang, but was nabbed after cops mounted a surveillance operation – with the court hearing how cops caught the gang with nearly £4m worth of drugs.

The drugs seized included 9.25kg of heroin, 8.25kg of cocaine, 12kg of amphetamine and £37,000 in cash.
